Apple Watch Series 12 and Ultra 4: hypertension notifications delayed in 33 regions

Apple’s watchOS feature-availability footnote says hypertension alerts may not ship at launch on Series 12 and Ultra 4 in 33 countries while new Health Sensing System clearances finish. U.S., UK, and EU are unaffected.

Apple’s watchOS Feature Availability page now carries an explicit footnote for Hypertension Notifications:

Hypertension Notifications may not be available on Apple Watch Series 12 or Ultra 4 in your region, as additional regulatory clearances for these devices are in process.

That warning is attached to 33 countries and regions, including Australia, Bahrain, Brazil, Canada, Colombia, Costa Rica, Ecuador, India, Indonesia, Israel, Japan, Kuwait, Malaysia, Saudi Arabia, Singapore, South Africa, South Korea, Taiwan, Thailand, Türkiye, Ukraine, the United Arab Emirates, and Vietnam (plus several smaller territories on Apple’s list).

Why Series 12 / Ultra 4 need new clearance

The new watches use Apple’s redesigned Health Sensing System — larger electrode surface area for ECG contact, ring-arranged photodiodes for optical sensing, and brighter green LEDs that measure heart rate every five seconds. Because the optical and electrical heart hardware changed, some regulators treat hypertension notifications on these models as a new clearance path even though the feature already exists on Series 9–11 and Ultra 2–3.

Apple and local reporting (including an Apple statement carried by PTI for India) say availability in affected markets is expected after those clearances finish — India-specific coverage points to next year, while Apple’s global footnote does not publish a single worldwide date.

Where the feature still works at launch

Countries without Apple’s Series 12 / Ultra 4 footnote keep hypertension notifications on the new hardware at launch. That group includes the United States, the United Kingdom, and EU member states, per Apple’s availability table.

Older compatible watches (Series 9 or later / Ultra 2 or later, excluding SE) are not covered by this Series 12 / Ultra 4 footnote — the delay is tied to the new models’ sensors, not a global kill-switch for the feature.
